Foreign doctors often have their education paid for, so they don't have the debt our doctors have. That means they live as well on less money coming in. Changes their way of looking at things.

      <![CDATA[There are certainly pros and cons on both sides of this argument.  <br>
I don't mind being treated for routine things by a physician's assistant or nurse practitioner, many practices have that going on already.<br>
<br>
As far as doctors go, not many people are going to go to through the labor of US medical school and come out with such debt only to be told how to practice medicine and be paid a pittance.  Foreign docs are such a good deal since they are happy since they are making so much more money than they would in their own countries even at reduced US rates.  <br>
<br>
So the effect of OCare is that we will now have exported jobs away from Americans.

      <![CDATA[I've always been a little suspect of why doctors wives know so much.  Do four years of medical school and residency transcend into a doc's wife mind?  I don't know of any bankers wives who can suddenly read a balance sheet or of insurance executives wives who can suddenly read mortality tables.  Why are these two so blatantly representing their husbands and not their constituent?

      <![CDATA[As far as medical care apart from regular checkups.  I've been so very fortunate through prostate cancer( 42 radiation treatments - no scarring or burning, still cancer free), acute sinus disease causing missing bone around my eye socket- fantastic Dr. at UAMS).  Sometimes it's what somebody knows to point you in the right direction, be it another doctor or a very knowledgeable civilian.  I'm very lucky to be married to one very knowledgeable civilian.

<br>
I don't have insurance, but I do have basic healthcare. It is easier to get an appointment with my doctor that it is with the nurse practitioner. It also costs less, but given the choice I would take my nurse practitioner.

      <![CDATA[cbb, Was $12,000 just your surgeon's fee? That seems over the top to me. My surgeon in Santa Monica only charges $4500 for a hip replacement using the anterior approach and he is probably in the top 3 of surgeons for that procedure in the U.S. He doesn't accept Medicare so he reduced his fee for me to $3500. It was worth it to be out of pocket that much as there was no PT and I was on a plane flying back to Arkansas in 8 days.

      <![CDATA[The supply of doctors is not great enough to meet the demand. Why you ask? Well, in part because the accrediting bodies have ensured that the numbers graduating from accredited medical schools - and nursing schools - has not grown proportionately to the population. How can this be, you ask? By controlling the number of qualified applicants who are admitted in the first place.<br>
<br>
And yet the conservatives tell us that competition can solve the problem.<br>
<br>
How can there be competition when there are not enough doctors to go around? <br>
<br>
Cost would go down if there were more providers, but the accrediting bodies have a vested interest in the continuation of an inadequate supply of providers.

      <![CDATA[The doctor who refuses to take Medicare and Medicaid insurance may become very lonely in the future as more and more companies dump their employee's insurance and the population gets older and older.  They are already having to shove though a patient about every 12-15 minutes to keep their income up and it is the insurors being pushed by the corporations to lower healthcare costs driving the program.  The problem will be the patient with no insurance that gets billed for the whole amount of the action, e.g., the $12,000 for my shoulder surgery while my bill (Medicare and supplemental) was discounted and co-payed and whatever down to $1,300 with no balance due (I do expect a balance due on the physical therapy).

      <![CDATA[Nurse practioners and physician's assistants are well qualified to order bloodwork, imaging and other tests. They are also well qualified to dispense medications. This last is what it's all about.  Big Pharma funds another study, and so ratchets down the "normal range" of lipids, glucose and the like.  This creates huge new markets of people who were fine yesterday, and are now suddenly "sick" today with a lifelong outlook of meds.  I have a friend who became a pharmacist, then an M.D., then did a lengthy residency to become a psychiatrist.  When I asked her what her practice was like, she said: "I give dope to bored housewives."  <br>
<br>
Surgeons and other specialists deserve the pay they get.  The days of the kindly old country doctor- and I am old enough to remember them- are long gone.  The modern analogue is only available in large metro areas; the "boutique" or "concierge" doctors with a client list of wealthy subscribers.

      <![CDATA[I don't believe there's a free standing doctor left in Fort Baptist. They've all been bought up by Sparks & St. Edwards. Everyone gets a Doc-in-the-box here even if there's few signs he's actually a Sparks Hospital employee. <br>

My new giant black doctor is very very nice. He's got a very nice car and wears only the finest suits and flies home to Africa to visit several times a year. I'm glad for him, but his doctoring reminds me of Lucy & Ethel wrapping those chocolate kisses coming down the conveyor belt. In yada yada blood test Out walk walk walk Wal-Mart pharmacy 4 dollars except for my insulin pens which went up to 285 dollars per filling. <br>

We got medical care too big to fail. But I'm still thankful I am now allowed to pay 255 dollars per month in order to buy two 4 dollar bottles of medicine though it only pays 20 bucks towards the 205 dollar box of insulin pens. Now why should I bitch?<br>
<br>
Obama threatens to pay medical providers less in the future which means old people will never find a doctor who'll accept Medicare patients....it's already happening to my older friends. I don't know the cure because ain't no doctor or hospital is willing to make less though now we'll endure months and months of well off people laughing at poor people hoping to make that 9 dollars per hour Obama promised. We're in a helluva mess.

      <![CDATA[There are a lot of people who go to the doctor routinely to have a BP check, follow-up on medication changes, assure that wounds are treated, and other activities that a PA or even an experienced nurse at a senior center or town center couldn't handle, leaving the doctor to have the time to deal with patients that actually require some review or diagnosis. Actually, it should be a given for post-surgical follow-up and the hospitals should be willing to pay for follow-up calls or visits to asure that everything is being taken care of, especially with significant penalities for re-admissions following surgery. Smart doctors. and I can guess where two of them might not be, work with PAs and nurses to do follow-ups to exyend their reach. <br>
<br>
Doctors who live in the past and want to continue pay for services need to move to a rich commune, i.e., Chenal or Maumelle or Beverly Hills but insurers will follow ways to reduce costs and they need to get in the plan or retire!

Massachusetts's health-insurance premium growth declined relative to the nation as a whole in the years since Romneycare has been enacted.  From 2006 to 2010, employer-sponsored health-care premiums for a family rose about 19% in Massachusetts, while they rose about 22% in the US as a whole.  Compare that to the period between 2002 and 2006, when Bay State family premiums increased 40% and US family premiums rose only 34.5%.  Family premiums have seen the greatest reduction in growth since Romneycare; individual premiums have also slowed their rate of growth, though by not as much.  For both family and individual premiums, the rate of growth fell below the national average in the period between 2008 and 2010.<br>

A physician's assistant is much, much more than a glorified nurse. My apologies to nurses because the vast majority of them are top notch and doing yeoman's work in the delivery of health care.<br>
<br>
My sister,  just this week,  chose a PA in Albuquerque to be her primary care provider. Her HMO did not renew their agreement with the doctor's group she was seeing. She had an issue that was urgent, but not an emergency. After going to a group that had a "Walk-ins welcome" sign and waiting 2 hours, she went elsewhere and found this lady and is very pleased. She won't be getting brain surgery from her, but was very satisfied that she is competent to handle most of her needs.

      <![CDATA[Two things, first I would rather be seen by a real doctor than a glorified nurse, and second according to friends who are doctors if their rates are cut much they will stop taking new Medicaid and Medicare patients. That will mean many people can't get healthcare. Pull you head out of your ass and realize these so called reforms will hurt care. Instead you simply hate drs because they make great money and have a good education.

Where was that AR doctor from, the one who netted over $800K last year, mostly from Medicare/aid, in a rural hospital?  Seems he'd be a perfect candidate for some NP competition.

      <![CDATA[It's always amazed me we don't talk about the way we deliver healthcare as well as the way we die. I don't know that PA's and nurse practitioners shouldn't play a larger role. Also we spend way to much on end of life care. In some situations the little death pill would be welcome as opposed to the agonizing, expensive Alternative.

      <![CDATA[Yes, if only Dr.'s had a strong lobby in the legislature, something good might happen to them.<br>
<br>
So...if I am understanding the R's correctly, you cannot tie a Dr.'s pay to his/her performance.  That would be silly, right?  Making the Dr. responsible for a patients bad choices and punishing the dr for that choice.<br>
<br>
Of course, the above is TOTALLY different in a school setting where one teacher's pay should be directly tied to the performance of her/his students.<br>
<br>
It would be such fun for a reporter to ask some of the R's about this.
