To those who asked, here is a link to his practice's FB page with their address and telephone number. You need to bring a current (As in past 60 days) report from your physician showing all your diagnosis, and you pay $250 For an office exam where he and his nurse go over your medical information, diagnosis, and overall health status, and they determine if you fit the guidelines for RX medical marijuana. If so he certifies that, and handles all the paperwork for your application for the prescription I'D, and gives it to you in an envelope addressed to the processing center, you include a cashiers check, money order, etc. For $50 To cover the application fee, and you are certified for a year after that. They are very knowledgeable and friendly, and do not mind explaining any questions you have especially if you have no experience with marijuana.
